2018 Ram 1500 Hemi Tick - Lifter or Exhaust Manifold?

Hey everyone,

My 2018 Ram 1500 Laramie with the 5.7L Hemi has developed a ticking noise, especially noticeable on cold starts. It seems to get a bit quieter once warmed up but never fully goes away. I'm worried it's the dreaded Hemi tick.

I've read a lot about it possibly being exhaust manifold bolts or lifters. The truck has about 85,000 miles on it. Has anyone experienced this with their 2018 Ram, and what was the fix? I'm trying to figure out if this is something I can check myself or if it's straight to the shop.

Summary: For 2018 Ram 1500 Hemi Tick - Lifter or Exhaust Manifold?, work through these steps on your 2016+ vehicle:

  1. Confirm the symptom — note when it happens (cold start, highway, after fuel fill, etc.).
  2. Scan codes — save all pending/stored DTCs; don't clear until you've recorded them.
  3. Check TSBs & recalls — look up your VIN for open campaigns and known fixes.
  4. Inspect the obvious — related fuses, connectors, fluid level, and recent repairs.
  5. Test the common fix — follow model-specific service procedures; one change at a time.
  6. Escalate if needed — burning smell, no brakes, overheating, or stalling in traffic → shop visit.
